The memorial is located at historic Columbus Fireman's Park, located on eastbound Park Avenue / Wisconsin Route 73 entrance to Fireman's Park, just east of its intersection with Ingalsbe Street, at 1049 Park Avenue, Columbus, Wisconsin 53925.

NOTE: Once inside the entrance, follow the one-way road (Veterans Memorial Way) until you see the children's playground on your right. The memorial is adjacent to the playground. (The memorial may not be easily visible when leaves are on the trees . . .)

Military Times: Honor the Fallen: Army Capt. John F. Kurth
Captain Kurth was 31 years old when the patrol he was traveling in was hit by an explosive devise in Tikrit, Iraq. He had been in Iraq less than a month when he was killed on March 13, 2004.

Find a Grave: Capt. John Frederick "Hans" Kurth
Capt. Kurth left behind a five-year-old son. He was awarded a Bronze Star and a Purple Heart. He is buried at Hillside Cemetery in Columbus, Wisconsin.
